People moving to the area

People moving into the area are welcome to register at this surgery. Please note that we only receive patients who are entitled to receive NHS treatment and whose permanent address lies within our practice catchment area

You can register at the practice by bringing your medical card to the surgery or signing and completing a registration form which we will supply. If you cannot come to the surgery, registration may be carried out by an authorised representative who may sign the registration on your behalf.

Doctors lists

Our reception staff will inform you which doctors' lists are open and you may express a preference for registering with a particular doctor. We do our best to accommodate your wishes but please be aware that we cannot register you with a doctor whose list is closed. We do encourage families to use the same doctor where possible. We feel this to be in your best interest.

New patient health check

When you register with the practice you will be invited to make an appointment for a new patient health check. This allows any significant health problems or family history to be recorded prior to the receipt of your medical record and allows you to discuss any issues or concerns you may have

Registration Forms

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice to save time completing it at the surgery. Please note: the Medical Questionnaire and Summary Care Opt Out form (if opting out) must also be completed for your application to be processed.
